The Gathering Place (Brattleboro Area Adult Day Services), is an Adult Day program serving Windham County Vermont adults with a wide range of physical and cognitive abilities. Our participants enjoy music, arts, games, exercise and companionship - just to name a few!
We are seeking a compassionate, organized, and community-minded Service Coordinator to join our dedicated Adult Day Services team.
Role
In this role, you will support participants, their families and caregivers, with a wide range of individualized services in our community-based environment:
Guide participants and their families through enrollment, ensuring necessary forms, assessments and authorizations are complete. Provide tours and follow-up on referrals.
Regular communication with families/caregivers on participant status and needs.
Assist in providing care, assessments, service coordination, benefits application, and planning assistance for other community supports.
Regular collaboration with community partners including case managers, housing supports, physician practices, transportation and participation with inter-agency team meetings.
Contribute to assessments and plans of service, discharge planning and documentation of services provided working closely with nursing and program management.
Actively participate in daily center activities including assistance with basic participant care (including ADL's), as needed.
Assist program staff and leadership as needed.
Coordinate annual satisfaction surveys and participates in QAQI activities.
Schedule: Full-time Monday-Friday, days (regular business hours), generally 8:45-4:45 with some flexibility. No weekends or evenings; we are closed major holidays.
Rate of pay: $21+ per hour depending on experience.
Benefits include up to 15 days paid time off in the first year (accrued based on hours worked) plus 7 paid holidays. We are unable to offer health insurance, however, dental and vision are provided at no cost. 403(b) match after one year.
Requirements
Minimum of 2 years in a human service role required, including at least one year working with older adults and/or adults with disabilities.
Experience with service coordination, case management, social work/assistance and/or knowledge of community-based services available to order Vermonter and adults with disabilities preferred.
Experience assisting with ADLs or providing direct care desirable.
Understanding of physical and cognitive needs of older adults.
Strong communication and interpersonal skills.
Organizational skills and the ability to prioritize tasks effectively.
Computer skills are required. Documentation and communication tools used are largely web-based.
