ECM Care Coordinator
A Greater Hope, the Inland Empire’s leader in community-based services for Medi-CAL families, is seeking a value-aligned advocate to support our Enhanced Care Management (ECM) program. This innovative service model is transforming the way communities care for our most vulnerable populations, and we need a compassionate innovator to support this new model.
The Care Coordinator will work with a multi-disciplinary team, holding a direct caseload as well as supporting colleagues to find and navigate resources for all ECM clients.
This role involves assessment, case planning and coordination, linking clients to all necessary services for whole-person care. Much of this role will be in-person, in the community.
Qualifications
- Qualified candidates should be highly organized and adaptable
- At least 4 years of experience (or 2 years with Associates Degree) in care coordination, case management, or healthcare services

- Strong communication and interpersonal skills to engage effectively with clients and care teams
- Proficiency in electronic medical records (EMR) systems and the Microsoft Office Suite

Requirements

- High School Diploma required, Associates Degree in health-related fields such as nursing, social work degree preferred.
- At least 4 years of experience (or 2 years with Associates Degree) in care coordination, case management, or healthcare services.
- Experience working with underserved or diverse populations.

Pay: $20.00 - $25.00 per hour
