About Your Role

The Care Manager Extender is responsible for assisting the Care Manager in delivering select Tailored Care Management services to clients with complex behavioral health and healthcare needs. The Care Manager Extender will comply with regulatory requirements and through service provision, will meet or exceed established key performance indicators and desired client outcomes. This position will report to the Care Manager.

Travel is required up to 75% or more of the time.

Do The Following Apply To You

  • High school diploma or equivalent
  • Bachelor’s Degree in social work, counseling, psychology or nursing (preferred)

Plus ONE Of The Following

  • Be a person with lived experience with an I/DD or TBI with demonstrated knowledge of and direct personal experience navigating the North Carolina Medicaid delivery system
  • Be a person with lived experience with a behavioral health condition who is a Certified Peer Support Specialist
  • Be a parent or guardian of an individual with an I/DD or a TBI or a behavioral health condition AND have at least two years of direct experience providing care for and navigating the Medicaid delivery system on behalf of that individual (note that a parent/guardian cannot serve as an extender for their family member)
  • 2 years of experience working with the MH/SU/IDD population
  • Have two (2) years of paid experience performing the following types of functions, with at least one year of paid experience working directly with the Tailored Care Management eligible population:
