What you will do:

As part of the Behavioral Health Leadership Team, this position will assist in efforts to ensure patient safety and promote high quality care throughout the department. This colleague will conduct regular audits of patient charts in accordance with NYS Office of Mental Health Utilization Review (UR) regulations to ensure that recipients are receiving appropriate services at an appropriate level of care. Additionally, this colleague will assist in the oversight of department incident reporting internally via MIDAS and externally via The Justice Center and Office of Mental Health. They will manage incident-related communications with internal and external agencies, lead incident investigations, facilitate the formulation of corrective action plans and conduct subsequent monitoring. They will conduct All Cause Analysis (ACA) and Root Cause Analysis (RCA) meetings to help identify discrepancies in generally accepted practice standards (GAPS) and work with appropriate teams to remediate identified issues. This colleague will collaborate with department teams on various quality improvement projects that align with goals of the department, system and state

Responsibilities:

  • Understands Utilization Review regulations for outpatient treatment settings.

  • Regularly collaborates with Medical Records to identify charts in need of review.

  • Completes thorough audits on outpatient charts, using established audit forms, to assess for compliance with admission, continued stay and meds-only policies, procedures and regulations.

  • Provides individualized feedback to clinicians and providers regarding non-regulatory quality items like general documentation practices, progression of care, safety planning, internal and external communication etc.

  • Returns completed audit forms to medical records and department leadership.

  • Escalates major deviations to department leadership to ensure prompt chart correction.

  • Re-reviews failed charts to ensure non-urgent deviations were corrected.

  • Maintains a comprehensive understanding of department policies and governing body regulations .

  • Understands internal and external incident reporting requirements and ensures timely and accurate reports are completed.

  • Manages incident-related communication and follow up with internal risk management, The Justice Center and NYS Office of Mental Health.

  • Completes thorough incident investigations per department policy and state regulation.

  • Facilitates Incident Review Committee (IRC) Meetings monthly to review and monitor our department’s reportable incidents.

  • Facilitate All Cause Analysis (ACA) and Root Cause Analysis (RCA) meetings to better understand incidents, their causes and ways in which identified discrepancies will be addressed.

  • Identify and communicate trends found across UR audits so corrective action plans can be considered.

  • Leads development of corrective action plans to remediate practice issues identified in incident investigations across the department.

  • Works collaboratively with department staff and leaders to develop and execute quality improvement initiatives.

What you will need:

  • Master's degree in Social Work is required.

  • Master’s degree in social work or master’s degree in counseling required. (MSW or MHC)

  • Current registration as licensed social worker or licensed mental health counselor required (LMSW, LMHC, LCSW).

  • Clinical license (LCSW) strongly preferred

  • At least 2 years of experience in an OMH licensed treatment setting

  • At least 2 years of experience providing quality assurance and/or clinical supervision in a behavioral health setting

  • Familiarity with behavioral health-specific interventions like psychosocial assessments, treatment planning and safety planning.

  • Familiarity with common psychiatric treatment interventions.

  • Experience navigating electronic health record (EHR) systems.

Pay Range: $30.00 - $43.08

Pay is based on experience, skills, and education. Exempt positions under the Fair Labor Standards Act (FLSA) will be paid within the base salary equivalent of the stated hourly rates. The pay range may also vary within the stated range based on location.
