Requirements for Licensed Social Worker:

• Must possess a minimum of a B.A in Social Work, LSW or equivalent degree with a current social work license.
  • Masters in Social Work (MSW), and LCSW or LICSW higher preferred.
• 1+ years skilled nursing facility experience preferred

• Job Duties of License Social Worker:

• Compiles social history data
• Participates in assessments, development and evolution of residents care plan.
• Responsible for planning and leadership interdisciplinary care planning weekly meetings.
• Documents timely progress notes according to state and federal regulations.
• Coordinates the discharge planning for residents.
• Able to interpret and implement established social service policies, procedures and regulations.
• Coordinates services of community agencies for continued delivery of care upon discharge.
