Requirements
Must be at least 21 years of age
This position requires a High School Diploma and at least four (4) years experience providing behavioral health services; or an acceptable and equivalent combination of education and experience
Must meet Agency requirements for Behavioral Health Technician
Intermediate computer skills. Working knowledge of MS office products including Excel and Access or similar software.
Must possess a valid AZ Drivers License
Must possess an AZ Fingerprint Clearance Card or be eligible for one

The general purpose of this position is to treat families under the 12 Principles and facilitate Child and Family Team Meetings.
- Responsible for the high needs case management of 25-34 service recipients and their families.
- Conducts the engagement process with families to identify natural/formal supports as outlined in the Arizona Model, Child and Family Team (CFT) process, and Regional Behavioral Health Authority (RBHA). Ensures families understand the behavioral health process, the role of CFT Facilitator, and ensure family participation that links them to natural or community based supports.
- Coordinates, schedules and facilitates CFT Service Plan meetings with natural, formal supports, and system stakeholders.
- With the assistance of the members of the CFT, completes the Strengths, Needs & Cultural Discovery (SNCD) reflecting the child/family's unique strengths, needs and culture. Develops an individualized Service Plan with collaboration of family and team members to reflect unique child/family needs, measurable objectives, action steps to meet the needs while incorporating the family vision and family strengths to meet identified needs/goals. Creates individualized service plans as identified by the CFT members according to the Arizona Model and RBHA guidelines.
- Monitors implementation of CFT/Service Plan and progress of service recipient toward goals as evidenced by thorough and comprehensive documentation, staffing notes, contact notes and other forms of documentation toward this goal. Submits documentation as outlined in agency standards
- Develops a comprehensive crisis and safety plan within the CFT
- Submits appropriate referrals to clinical services within agency and to other community agencies identified by CFT.
- Updates and maintains electronic medical records on each case which include but not limited to; CFT Service Plan, RBHA demographics, and CALOCUS according to RBHA and Arizona Department of Health Services (ADHS) guidelines.
- Participates in court hearings for service recipients when necessary
