Job Summary

The Case Manager provides support to Oakland County residents at the Oakland County Resource and Crisis Center (RCC). This position is a full-time office position which requires minimal travel. The primary focus of the case manager position is to provide support, education, connection to community resources and advocacy to persons served. Some major roles and responsibilities include Michigan Department of Health and Human Services (MDHHS) processes, various types of employment advocacy, and familiarity of the services provided by OCHN including crisis services at the RCC.

Essential Functions

  • Be familiar and comfortable with the culture and characteristics of persons with substance use disorders and Mental Health disorders and provide a welcoming and accepting environment in which this population and their families can receive services.
  • Completes documentation and required paperwork within 24 hours of service delivery.
  • Have knowledge of the Michigan Department of Health and Human Services (MDHHS) programs and eligibility requirements.
  • Have knowledge of the MDHHS MI Bridges website and services available to clients through it.
  • Be familiar with community resources available to Oakland County residents.
  • Have knowledge of Oakland Community Health Network (OCHN) processes and help clients navigate mental health and substance use screening parameters.

Education

  • Bachelor's degree in the human services field (Psychology, Counseling, Criminal Justice, etc).


Training Requirements (licenses, programs, or certificates)

  • Human Service License.
  • CADC or CAADC preferred.
  • Mental Health and/or Substance Use Disorder experience.

Experience Requirements

  • At least one year working in the human service field. School internships may be considered.

  • Have knowledge of health insurance practices and be able to direct clients in using services available to them through their health plans.

Knowledge Requirements

  • Knowledge of protocols related to acute care and transitions in care.
  • Knowledge of services and support in a crisis continuum of care.
  • Knowledge of the Petition and Certification processes.
  • Knowledge of Recipient Rights.

Additional Information

  • Must have available means of transportation to and from OCHN and for required offsite meetings or site visits.
  • Must be available for meetings and events which may occur outside of standard office hours.
  • Work performed primarily in an office environment.
