We are seeking a Support Coordinatro to join our independently owned and operated national agency. This individual must be eager to make a difference and be willing to be cross trained in other areas of the business.
Care Manager Benefits include:

Care Manager Primary Responsibilities:
- Schedule caregivers with clients and communicate to all parties
- Conduct/train Article 9 and prevention and support classes to all employees
- Conducts daily/weekly reporting
- Conducts quality calls and quality visits
- Confirm and update caregiver availability including vacation, sick time, and holidays
- Varifies payroll daily
- Establishes and build realtionships with contract providers to gain new clients and understand their needs
- Respond to employment leads regarding open positons with possible clients
- Document skills and attributes of caregivers to ensure strong matching to clients
- Follow up with new caregiver hires to explain the scheduling process
- Establish a great rapport with caregivers and clients
- Obtain ISP updates and enter HAB goals as per the ISP
- Ensure that authorization is in place prior to services starting
- Is on call as per on call calendar requirements
- Follow up on all new leads, track needs and staff appropriately
Care Manager Requirements:
- Experience – One year experience in a home care setting preferred, and understand the DDD industry, and be willing to obtain Article 9 instructor certifications
- Education – Associates degree in any field
- Skills – Exceptional customer service skills, time management skills, excellent communication skills, willingness and able to become if not already an Article 9 and prevention and support instructor. Must be able and willing to train, mentor, interview and hire all new team members.
- Characteristics – This is an autonomous position, so we’re looking for individuals that are self-motivated and willing to learn.
