Social Worker – Skilled Nursing Facility | Bronx, NY

A well-established Skilled Nursing Facility in the Bronx is seeking a dedicated, energetic, and professional Social Worker to join our compassionate clinical team.

Position Overview:
The Social Worker will play a vital role in supporting residents by identifying and addressing their psychosocial, emotional, and mental health needs. This includes facilitating access to appropriate services, advocating for resident rights, and ensuring compliance with all relevant federal and state regulations. The ideal candidate will help foster an environment that promotes dignity, autonomy, and quality of life for every resident, while contributing to a culture of respect and individualized care.

Key Responsibilities:

• Conduct psychosocial assessments and develop care plans in collaboration with the interdisciplinary team.

• Assist residents and families with adjustment to the facility and support during transitional periods.

• Coordinate community resources and services to meet individual resident needs.

• Promote a positive and inclusive atmosphere that enhances residents’ well-being.

• Collaborate with the Admissions Department and participate in outreach and marketing efforts to strengthen relationships with referral sources.

Qualifications:

• Bachelor’s degree in Social Work (BSW) or a related human services field (required).

• Prior experience working with elderly populations in long-term care or community settings (preferred).

• Strong communication, assessment, and interpersonal skills.

• Ability to work both independently and collaboratively within an interdisciplinary team.

• Experience in marketing and admissions-related functions is a plus.
