You’ll work in a flexible, patient-focused environment that empowers you to manage your own schedule while making a meaningful difference.Key ResponsibilitiesCoordinate with interdisciplinary team members to manage and deliver individualized patient care plansProvide social work support to patients in Lansing, MI and surrounding communitiesFacilitate referrals to supportive services and ensure patients' social, emotional, and financial needs are addressedMaintain accurate, timely documentation using EMR systemsUphold patient confidentiality and foster strong communication with both the care team and familiesTravel to and from patient homes within the local territory as neededMeet charting deadlines and manage daily workflow independentlyIdeal Candidate ProfileMaster’s in Social Work from an accredited graduate program (Council on Social Work Education)Minimum of 1–2 years post-graduate experience in a healthcare environmentActive and valid MSW license in the state of MichiganSkilled in using EMR systems, mobile devices, and tablets for clinical documentationOrganized, self-directed, and comfortable working autonomouslyExcellent interpersonal and communication skills with a commitment to high-quality patient careAbility to travel daily (~20% of work time) across the service territory
