As a Resident Care Manager (RCM) - RN or LPN with previous leadership experience

Looking of for a Nurse to join our team as our Nurse case manager for our small Memory care unit that is located within a SNF Skilled Nursing Facility setting. Are you a Nurse with passion for Alzheimer’s, Dementia, and related disorders conditions? Do you want to make a difference in the lives of others? Your experienced nursing skills will be utilized in t our Five-Star Resident-Centered quality of care facility working closely with residents, their families, front line nursing staff, Physicians, healthcare providers and the facility inter-disciplinary teams.

Qualifications

• Must be a licensed RN or LPN for the State of Oregon with strong assessment and care planning skills.
• 2 - 3 years experience working as an RN or LPN.
• MDS experienced preferred, but not required
• RNs with some long-term care experience, a strong passion for working with the senior population, and experience in Nurse Supervisor roles are encouraged to apply.
• You will receive excellent, hands-on training to learn Marquis nursing systems and procedures.
