Summary:The Care Advocate is a direct service role whose primary function is to assist clinical & health service departments to provide individual attention to clients, crisis prevention and intervention, monitor clients, assist in ADLs and encourage wellness through participation in their treatment plan goals. The Care Advocate may also be responsible for assisting in other departments as assigned. Qualifications:Must have the following:
- HS Diploma or Equivalent
- Associates or bachelor’s degree in a human service field preferred
- Experience working with adults who have serious mental illnesses and/or substance use issues
- Training, as prescribed by the Illinois Department of Human Services Division of Mental Health prior to assuming the duties of this position (or within the last 5 years)
- Free of a criminal background that is a Disqualifying Offense as defined by the Illinois Department of Public Health Skills Required
- Able to read, write and speak English
- Ability to build rapport with clients of different genders, ages, races, and cultural backgrounds
- Ability to manage multiple tasks at once
- Ability to prevent and de-escalate crisis situations
- Strong professional boundaries
- Able to identify symptomology
- Able to communicate effectively with the IDT (interdisciplinary team) Essential Duties:
- Monitor the milieu and seek opportunities for interaction and engagement with clients
- Model pro-social behavior and instruct clients on the same
- Assist with room checks to identify potential risk issues and to engage clients who are prone to staying in their rooms all day
- Bring to the attention of the IDT any issues with activities of daily living (ADLs)
- Instruct clients on proper hygiene and cleanliness
- Communicate any abnormal behaviors or medical issues to the appropriate staff
- Monitor the floor to identify any safety issues. Fix the issue when possible, if not able, communicate to the person in charge
- Monitor the medication line, lunch line, and patio to prevent and manage conflicts that may arise
- Facilitate assigned activities/services for clients
- Assist clients to get to their assigned groups and medication pass
- Document their efforts in the client chart, or in the manner dictated by the facility
- Participate in quarterly staffing as assigned
- Must attend scheduled clinical supervision
- Other duties as assigned
