Summary/Objective

Primary job responsibilities include supervising and managing Care Coordination staff providing care coordination and behavioral health services at Boundless Behavioral Health. Working with clients, parents/guardians, staff, and community partners, the Care Coordination Manager facilitates access to care and addresses barriers. The Manager will provide counseling, support, care coordination, and linkage both within Boundless service lines and with outside providers in order to promote meaningful treatment and improve outcomes for the individuals served. This position reports to the Director of Outpatient and Community-Based Services and works closely with all other departments. This is a full-time, salaried position.


Essential Functions

  • Identifies and assesses psychosocial and life skills needs through analysis of factors impinging on client adjustment including factors in the home and community.
  • Provides parent/guardian informational workshops on system navigation, client rights, and other information that assists in access to services and supports.
  • Conducts staff development/training on funding, system navigation, treatment options, changes within the Ohio funding system, etc.
  • Provides counseling to clients in the Outpatient Clinic, supported living, and the pathways program as needed.
  • Conducts staff development on issues related to social-emotional-environmental factors that impact treatment.
  • Provides work supervision for all care coordinators in individual and group settings.
  • Supports management of the outpatient clinic in collaboration with other department managers.
  • Works closely with Intake department, psychiatry, clinicians, psychology, and other internal teams to promote coordination of care and integration of clinical services.
  • Maintains all required clinical records and submits appropriate documents for statistical reports with adherence to program standards in behavioral health.
  • Attends meetings and professional development activities as required.
  • Monitors budget and productivity for staff.
  • Facilitates and supports the growth and development of Boundless services; as well as supports integration of services across the agency.
  • Provides care coordination support to consumers including community psychiatric support and therapeutic behavioral support to promote the coordination and implementation of treatment goals.
  • Serve as a liaison between families and agency to positively promote collaborative processes in the treatment planning for clients by encouraging parent/guardian participation in treatment.
  • Locate and mobilize community resources to support the treatment.
  • Manages daily functioning of care coordination services, including staff including staffing and assigning referrals.
  • All other assigned duties.

Education/Experience:
  • Master’s degree in social work or counseling, currently licensed LISW-S/LPCC-S in the State of Ohio.
  • Three years of experience providing case management services preferred.
  • Previous supervisory experience preferred.
  • Experience working in the behavioral health field required.

Skills, Knowledge and Abilities:
  • Excellent communication skills, both oral and written, demonstrating professionalism with both co-workers and parents and consumers
  • Excellent organizational skills with the ability to stay focused
  • Ability to prioritize multiple tasks
  • Demonstrated clinical knowledge of children and adults with co-occurring developmental disorder and complex behavioral health issues
  • Supervisory experience
  • Experience working within Electronic Health Record systems
  • Must be able to show tact and diplomacy when dealing with parents, clients, peers and staff

Additional Eligibility Qualifications
  • Must be able to function independently and have flexibility, personal integrity and the ability to work effectively with residents, staff and support agencies.
  • Must be able to relate to and work with ill, disabled, emotionally upset and at times hostile people within the programs.
  • Must possess the ability to make independent decisions when circumstances warrant such action.
  • Must be able to adjust to a flexible work environment, which includes working in homes, the community and an office.
  • Transports clients as needed; operates own vehicle
  • Required valid Ohio Driver’s License with Ohio Bureau of Motor Vehicles; proof of auto insurance; insurable driving record (five points or fewer)


Supervisory Responsibility:

Provide supervision to Care Coordinators.

Work Environment:

This job regularly works in the community (Delaware, Knox, Licking, and Morrow Counties), providing services outside of the office, in schools, client homes, and other community settings. This role routinely uses standard office equipment such as computers, phones, and photocopiers. (This job is hybrid - 85%/15% remote and in the community.)

The employee frequently is required to stand; walk; use hands to finger, handle or feel; and reach with hands and arms.

Position Type/Expected Hours of Work:
This is a full-time position, and hours of work and days are Monday through Friday, 8:00 a.m. to 4:30 p.m. Occasional evening and weekend work may be required as job duties demand.

Travel:
Travel is primarily local during the business day, although some out-of-the-area and overnight travel may be expected on occasion. This position does require regular use of a personal vehicle for local travel to meetings and appointments in the community.
