Role Description

This is a full-time on-site role as a Resident Care Coordinator located in Provo, UT. The Resident Care Coordinator will be responsible for care coordination, case management, communication with residents and families, care services, and delivering exceptional customer service on a daily basis.

Qualifications

• Care Coordination, Case Management, and Home Care skills
• Excellent communication and interpersonal skills
• Strong customer service orientation
• Experience in a healthcare or senior care setting
• Ability to work collaboratively in a team environment
• Knowledge of Medicare and Medicaid regulations
• CNA certification is preferred
