The Adult Outpatient R.E.A.C.H. Program provides comprehensive care and support for individuals trying to overcome significant life challenges. This program focuses on developing and enhancing individual social and interpersonal skills, employment skills and job development, educational functioning, participation in group exercises, and diminishing tendencies towards isolation and/or withdrawal. The goal of the program is to increase patient independence and effective lift management.

JOB SUMMARY
The Care Manager participates as a member of a multidisciplinary treatment team to provide advocacy, coordination, and continuity of evidence-based care for individuals and families in need through a collaborative process of triage, assessment, planning, and communication.
JOB DUTIES & COMPETENCIES
- Completion of thorough assessments and coordination of community resources based on patient needs as per program requirements.
- Complete all clinical and administrative documentation based on established timelines as per program requirements.
- Develop service plans, treatment plans, and/or aftercare plans with clear goals and objectives as per program requirements
- Communicate effectively and maintain cooperative relationships with internal and external resources.
- Maintain efficient workflow management and organization as per program requirements.
- Maintain proficient quality of clinical documentation as per program requirements.
- Other duties as assigned or needed as per program requirements.
JOB REQUIREMENTS
- Bachelor’s degree in a Human Services related discipline
- A minimum of 1-year experience in the mental health field is preferred.
- Must be a minimum of 21 yrs of age with a valid FL drivers license
- Ability to read, analyzes, and interprets clinical records, procedures, or governmental regulations.
- Ability to write reports and document clinical activities and effectively communicate verbally with, provide information to, and respond to questions from managers, patients, service providers, managed care entities, family members, and other stakeholders.
