REQUIREMENTS:
1. Must have a Bachelors Degree in social work (BSW) from a school accredited by the Council on Social Work Education.
2. Must be knowledgeable about the psychosocial and medical aspects of hospice care.
3. Must have a sincere interest in the hospice concept.
RESPONSIBILITIES:
Determine patient eligibility for hospice care along with Medical Director, Executive Director, and Patient Care Coordinator. Conduct initial assessment of the psychosocial needs of the patient/family and give explanation of the hospice program. Complete psychosocial assessment from upon patient's admission. Explain the following forms and see that they are properly signed and witnessed.

Home Visit Consent Form
Authorization for Assignment of Insurance Benefits - (if applicable)
Medicare/Medicaid Election
Medicare Secondary Payor
Client/Family Information
Advanced Directive Checklist
Provide Family information regarding Advanced Directives. Develop a psychosocial and spiritual careplan on the patient and maintain throughout patient/family enrollment in program. Complete psychosocial assessment and make sure all required forms are signed and careplans have been developed. Provide counseling and support once a month and PRN according to needs; complete progress note on all contacts. Work closely with Patient Coordinator to assess and meet needs and concerns. Provide psychosocial support to hospice staff as needed. Attends Interdisciplinary Team meeting once a month to assist in evaluation and revising patient/family's psychosocial and Spiritual careplan for patient/family. Family contact and support at death. Make bereavement assessment contact visit within one month after client's death. If families elect the bereavement program, schedule contacts monthly or as needed. Families and significant others may remain active in Bereavement Services for two years. Bereavement Group meetings are conducted monthly. Hospice Satilla conducts Bereavement meetings for community and hospice deceased clients family. Conduct volunteer inservice when volunteer coordinator is not available. Works closely with the Hospice Counselor or other Hospice employees to ensure patient and family psychosocial needs are met.
