The Intake Coordinator obtains vital statistics included in starting the entry process and organizing the initial database for the ICD patient chart.
Responsibilities:
A day in the life of an Intake Coordinator at Hackensack Meridian Health includes:
  • Schedules patient appointments in Cadence (patient scheduling module in EPIC).
  • Answers phone calls from patients, physician's offices and other agencies and returns calls to same.
  • Makes phone calls to patients' families at request of physician/clinician to obtain additional information, provide feedback and/or change appointment date/time as needed.
  • Pulls charts for upcoming appointments and creates new charts for new patients as needed.
  • Obtains and enters all needed information for insurance or triage.
  • Educates and assists families with their individual insurance policies and procedures.
  • Files and faxes documents.
  • Adheres to the standards identified in the Medical Center's Organizational Competencies.
Qualifications:
Education, Knowledge, Skills and Abilities Required:
  • High School diploma, general equivalency diploma (GED), and/or GED equivalent programs.
  • Excellent phone skills.
  • Excellent customer service skills.
  • Excellent organizational skills.
  • Knowledge in Microsoft Office (Word, Excel, Outlook).
Education, Knowledge, Skills and Abilities Preferred:
  • Bilingual (English/Spanish).
Licenses and Certifications Required:
  • Epic Cadence within 90 days of entering position.
Contacts:
  • Regular contact with Medical Center personnel, patients and visitors.
Working Conditions:
  • Lifts a minimum of 5 lbs., pushes and pulls a minimum of 10 lbs. and stands a minimum of 1 hour a day.
