About Job
Developing care plans based on clinical goals and needs assessments
Administering medications, including injections and IVs, as directed by the physician
Providing support to families of residents throughout the end of life process, including grief counseling and helping family members make decisions regarding patient care
Identifying residents' psychosocial, mental and emotional needs
Providing, developing, and/or aiding in the access of services to meet those needs.
Assist with Discharge planning.
Requirements: Bachelor's degree in social work or related study.
