Your Responsibilities
As aCommunity Integration Coach,your primary responsibilities include providing case coordination, skills teaching, counseling, crisis intervention and support services to clients in the Hillcrest Supported Living Program. In addition, you will be:
Hillcrest Supported Living (HSL)is a program that provides individuals in developing theskills they want or need in order to gain satisfaction in their lives, and remain successfully employed. Services are provided in the community. OurCommunity Integration Coach provide skill-building and training in all areas of daily life, including:
Expected results of the Supported Living include the development of a support network in the community, successful residence in the community, successful employment and the prevention of unnecessary hospitalizations.

Description
Your Responsibilities
As aCommunity Integration Coach,your primary responsibilities include providing case coordination, skills teaching, counseling, crisis intervention and support services to clients in the Hillcrest Supported Living Program. In addition, you will be:
- Providing case coordination for each client
- Conducting assessments and creatingservice plans based on client needs, wants, and abilities
- Assisting in obtaining appropriate community resources or services for each client
- Establishing and maintain funding for each client
- Maintaining a complete and updated clinical record for each assigned client
- Assisting clients in developing or enhancing the skills and supports necessary to be successful and satisfied in the role recovery environment of their choice
- High school diploma or equivalent
- 2 years experience working with adults with disabilities
- Must be able to drive agency vehicles
- Must have access to a reliable vehicle and it should be insured
