Overview

We are seeking a dedicated and compassionate Social Worker (Master’s Degree) to join our team. The Social Worker works directly with foster care youth (18 yrs – 21 yrs) to support them in all life domains that prepare them for independence and self-sufficiency. The Social Worker has a small caseload of 12 youth who are living in our residential homes throughout Riverside, who are either attending school and/or employed. This is a full-time hybrid position that includes a remote work of home and on-site schedule. Our office is located close to the Mission Inn in downtown Riverside and the housing sites are all within a few minutes of this location.

The Social Worker is responsible for providing 24/7 after-hours crisis assistance to youth as needed. This is not a common occurrence, but it does require assuming on-call responsibility.

Responsibilities

  • Use a strengths-based approach with youth in creating a Transition Plan and developing and monitoring progress of monthly goals.
  • Serve as primary contact for Child Welfare Workers and Probation Officers
  • Conduct a pre-placement appraisal with the placing agency and NMD. Explain the program expectations and complete necessary intake paperwork
  • Be knowledgeable of transitional housing programs and resources for transitional aged foster youth.
  • Be knowledgeable in the Community Care licensing regulations for THP- NMD
  • Conduct monthly Satisfaction Surveys with youth and their placement workers
  • Facilitate conflict resolution plans and training with youth
  • Respond to complaints made by youth or concerns from the placement workers
  • Provide crisis intervention and management services to youth experiencing emergencies or difficult situations.
  • Conduct timely interviews for the THP-NMD and intake assessments
  • Utilize motivational interviewing techniques to empower youth in making positive changes.
  • Complete Transition to Independent Living Assessments semiannually.
  • Participate in CFT meetings with youth and their placement workers
  • Provide at least weekly consultation to the NMD. Consultation may but is not limited to discussion of progress towards goals, teaching life skills, teaching parenting skills, public benefits advice, computer training, transportation assistance, assist in contact with appropriate supportive adults/mentors.
  • Provide assistance with managing the NMD's cash resources, personal property, and valuables.
  • Provide assistance with maintaining accurate records of the NMD's bank
  • account, savings, and monthly budget.
  • To provide and/or coordinate referrals for additional services, consultants and
  • community resources as needed (i.e. medical, therapy, psychiatric, parenting
  • classes, job training, etc
  • Document the NMD weekly progress at home, school, and/or work, major incidents, and other significant, collateral contacts
  • Complete special incident reports and any other reports as required by the placing agency.
  • Maintain contact with placement worker and inform of any significant events/incidents
  • Consult supervisor regarding incidents, significant issues, and possible terminations, and follow-up as directed
  • Ensure that youth files are complete and up to date.
  • Submit all reports and documentation to supervisor for final approval.
  • Facilitate trainings, and support groups as directed by supervisor.
  • Input notes into an Electronic Health Record portal for ECM (Enhanced Care Management)
  • Performs other duties as assigned or requested

Requirements

  • Master's degree in behavioral sciences or human service.
  • 3+ years of experience providing case management to transition-aged youth, foster and/or probation youth or supervision of case management staff.
  • 2+ years of experience in an Administrative or Managerial position.
  • Valid California Class C Driver's License.
  • Valid automobile insurance.
  • Physical ability to operate a motor vehicle.
  • Physical ability to perform Pro-ACT and administer First Aid/CPR

Salary: $70,000-$85,000 annually

Job Type: Full-time

Pay: $75,000.00 - $85,000.00 per year
