Schedule: Monday–Friday, 9:00 AM–5:30 PM with occasional on-call rotation

Position Summary:
We are seeking a dedicated Master’s-level Social Worker (MSW) to join our interdisciplinary hospice care team. The ideal candidate is empathetic, organized, and committed to providing emotional, psychosocial, and practical support to patients and families navigating serious illness and end-of-life care.

Key Responsibilities:

• Provide psychosocial assessments and develop individualized care plans
• Support patients and families with end-of-life planning, grief counseling, and community resources
• Collaborate with nurses, chaplains, physicians, and other team members to ensure holistic care
• Assist with advance directives, funeral planning, and support around difficult decisions
• Offer crisis intervention and bereavement support as needed
• Maintain accurate and timely documentation in accordance with regulatory standards
• Participate in team meetings and case conferences

Qualifications:

• Master’s degree in Social Work (MSW) from an accredited program
• At least 1 year of experience in hospice, palliative care, or a related healthcare setting preferred
• Strong communication and emotional resilience
• Comfortable working in homes, facilities, and hospitals as needed
• Valid driver’s license and reliable transportation required
