If Yes, Here’s How You Will Contribute to the Team:

As a Patient Care Coordinator, you will:

  • Coordinate services for patients transitioning from hospital or related facilities to home health care.
  • Support the development and implementation of patient care plans post-discharge, ensuring alignment with physician directives and agency requirements.
  • Collaborate with hospital social services and discharge planning teams to facilitate home health services.
  • Ensure adherence to regulatory requirements related to admissions and referrals

  • Manage the admission process with referral sources and the sales team.

  • Obtain necessary authorizations for home health services from insurance provide

  • Assign caregivers to patients based on care plans and service requests.
  • Provide caregivers with accurate instructions and information regarding patient needs.
  • Report scheduling changes to the Director of Intake & Staffing and notify patients of any modifications to their care schedule.
  • Assist in monitoring staff performance, including dependability and responsiveness
  • Maintain accurate and up-to-date records of patient services, staffing, and scheduling.
  • Complete and manage documentation, using both paper and electronic systems.
  • Monitor overtime and mileage records, and address emergency scheduling needs as required.
  • Provide timely notifications of schedules and changes to employees, patients, contract agencies, and supervisors.
  • Address and resolve issues related to patient care and staff scheduling.
  • Adhere to HIPAA regulations to ensure patient confidentiality and privacy

Must Have:

  • High school diploma or equivalent
  • 1 year of scheduling experience in home health, skilled nursing facilities (SNF), or a healthcare setting.
  • Active Certified Nursing Assistant (CNA) certification in the state
  • Willing to work beyond normal working hours, weekends, holidays, and/or on other shifts and in other positions when necessary
  • Must be knowledgeable of nursing practices, procedures, terminology, laws, regulations, and guidelines that pertain to long-term care
  • Knowledge of Medicare rules and regulations, including JCAHO standards

Related experience in a long-term care, healthcare, or skilled nursing environment
