JOB TITLE: IICAPS Mental Health Counselor

DEPARTMENT: Community and Home-Based Services

SUPERVISED BY: Director of Community and Home-Based Services or a designee.

Job Summary

As part of a team, provides in-home mental health services to children in a variety of settings. Supports Clinician in providing an array of interventions including therapy, crisis response, intensive case management, consultation, advocacy and linkages. Interfaces with other professionals to provide collaborative care. Adheres to IICAPS credentialing requirements

Duties And Responsibilities

  • Is responsible to the IICAPS Director and any other supervisor the Director assigns.
  • Complies with all Yale IICAPS' requirements for model fidelity.
  • Collaborates with the IICAPS Clinician in assessment and treatment planning. for children, parents and families,
  • Offers therapeutic support and parent guidance to children and their families in a variety of settings (home, community, school, clinic).
  • Provides an array of therapeutic interventions (individual, parent family, crisis response, child specific school consultation, case management, advocacy and concrete service delivery).
  • Maintains clinical records within the IICAPS web-based system and CCGC's Credible web-based electronic health record.
  • Meets deadlines for completing documentation.
  • Participates in supervision and case presentations.
  • Interfaces with CCGC medication prescriber and families.
  • Manages ongoing insurance authorizations for cases.
  • Is expected to be involved in special projects, such as consultation, prevention programs and community education.
  • Responsible for other jobs that may be assigned by a Director or Executive Leadership.

Qualifications/Licensure

  • Bachelor's degree, required, preferably with a Human Services, Psychology or Counseling major. Prior experience in human services, mental health, or education field.
  • Valid Driver's License with reliable means of transportation.

Credentialing

This person is credentialed to provide rehabilitation services to children 0 to 19 and their families under the supervision of a licensed clinician
