Social Worker – Case Manager

This position is partially or fully grant funded. Employment beyond the term of the grant award may be subject to approval by Executive Council.

About the Job:

Provides support, advocacy, and helps to develop holistic plans of care for children with autism or suspected autism. Serves in a case manager role to promote the psychosocial, mental, emotional and cultural well-being of children and families. Recognizes the strengths and wishes of each child/family, working to maximize individuality, independence and dignity.

Here’s what you’ll be doing:

• Lead a team with you and a community health outreach worker to develop and implement holistic and family led care plans for families impacted by autism (or suspected autism).
• Facilitate and compile additional training in family advocacy, care for persons with special needs, and other training as appropriate.
• Understand and meet all regulatory requirements for social services delivery and documentation.
• Work with the interdisciplinary team and administration to promote and protect the rights and psychosocial well-being of each patient and advocate for each family.
• Develop protocols, procedures and a framework for a community health model of care delivery for families impacted by autism.
• Develop community relationships and partnerships to understand available services for families, and to advocate for these services.
• Work with patients, families, significant others and care teams to provide support, information and organization.
• Take a proactive role to improve the quality of life of the residents and their families.
• Complete social histories, psychosocial assessments, individualized care plans, and required documentation in a thorough and a timely manner.
• Lead and participate in all Care Conferences.
• Provide culturally safe care, striving to support each family within the framework of their spiritual and cultural practices.

What you’ll need to be successful:

• Bachelor’s degree in Social Work (BSW) or related social science field required, Master’s degree preferred.
• One year experience working in an educational or mental health field required, with preference given to work with children with disabilities
• Familiarity with social service, school and health care systems
• Must have and maintain a valid driver’s license, be at least 18 years of age, and be eligible for coverage under the Tribe’s Insurance
• Must successfully pass all applicable background checks and drug screens
• FCP complies with all CDC vaccine and testing recommendations for healthcare professionals.

• Experience with community advocacy
• Experience with grant development, budgeting and reporting
• Leadership and project management experience
• Familiarity with healthcare policies and protocols
• Knowledge of local social services framework
