The Behavioral Health Care Manager plays a key role in delivering comprehensive behavioral health services, including diagnostic assessments, symptom monitoring, and ongoing care management. This position involves providing psychoeducation, patient education tools, and brief interventions—either via phone, in-person, or telehealth—as clinically indicated. As a vital member of the collaborative care team, the Behavioral Health Care Manager works closely with the patient’s primary care provider, a psychiatric consultant, and a multidisciplinary care management team to support integrated, patient-centered behavioral health care.

How you can make a difference

  • Provide screening, diagnostic assessments, and treatment recommendations for common mental health and substance use disorders.
  • Perform co-visits with primary care providers and clinical staff as needed.
  • Engage with patients in clinic, telehealth, telephone, and/or via secure patient portal.
  • Create and coordinate patient-centered and team-based treatment plans, including physical and behavioral health symptoms and goals.
  • Support psychotropic medication management as recommended by psychiatric consultants and prescribed by treating providers, focusing on treatment adherence, side effects, psychoeducation, and monitoring effectiveness of treatment.
  • Perform risk assessments, safety planning, and crisis intervention.
  • Create and manage clinic-specific patient registry, including tracking patient contacts and clinical outcomes, as well as using registry to identify and re-engage patients.
  • Document patient progress, treatment recommendations, and interventions to share with treatment team.
  • Provide patient education regarding common mental health and substance use disorders, as well as available treatment options.
  • Provide brief interventions using evidence-based techniques such as Motivational Interviewing, Behavioral Activation, and Problem-Solving Therapy as clinically indicated; Clinician will also employ IPT, CBT, DBT, and other evidence-based techniques as appropriate.
  • Prepare for and participate in weekly patient systematic case review with consulting psychiatrist and communicate treatment recommendations to patient’s treating provider. Preparation for systematic case review involves running reports and gathering necessary information.

  • Ability to conduct thorough screenings and diagnostic assessments for mental health and substance use disorders.
  • Proficient in identifying risk factors, safety planning, and responding to crises effectively.
  • Skilled in developing patient-centered, integrated care plans in collaboration with multidisciplinary teams.
  • Strong communication skills to work effectively with primary care providers, psychiatric consultants, and care teams.
  • Ability to build rapport and engage with diverse patient populations via in-person, phone, telehealth, or secure messaging platforms.
  • Skilled in translating psychiatric recommendations into clear, actionable steps for patients and providers.
  • Experience managing and utilizing a patient registry to monitor progress, re-engage patients, and track outcomes.
  • Accurate and timely documentation of assessments, interventions, and treatment plans for coordinated care.
  • Ability to monitor treatment adherence, side effects, and overall progress using both clinical judgment and data tools.
  • Comfortable providing services through virtual platforms and adapting care strategies to remote modalities.
  • Flexibility to work in fast-paced, collaborative environments and adjust to evolving care models.
  • Commitment to evidence-based practice and staying current with advancements in behavioral health care.

Experience to drive change

  • Master’s degree in social work along with current state license (LMSW) required
  • 3+ years of experience in a health care and/or behavioral health setting
  • Training in Collaborative Care Model (CoCM) & 1+ year experience providing CoCM services highly preferred
  • Foundation of social work ethics that informs a thoughtful, evidence-based approach
  • Utilizing a variety of electronic health records including data capture, data mining and reporting
  • Ability to independently conduct psycho-social assessments and provide treatment to a wide variety of individuals from various socio-economic, cultural, ethnic, educational, and other diverse backgrounds.
  • Knowledge and experience in the use of medical and mental health diagnoses (DSM-V), disabilities, and treatment procedures.
  • Independent judgment and skill in utilizing therapeutic (CBT, Motivational Interviewing, Behavioral Activation), supportive, problem-solving, or crisis intervention techniques.
  • Experience using MS Office (Word, Excel, etc.) and data management software.
  • Excellent communication skills, both verbally and written
