Position Description:

At Stony Brook Eastern Long Island Hospital, our Social Workers/Mental Health Counselors are valuable members of our team, who provide empathic care to our patients, their families and visitors, with the highest degree of integrity and respect. Social Workers/Mental Health Counselors apply the principles and practices of their profession to provide integrated social casework, including direct counseling, therapy, advocacy, and other services to individuals, families and groups in a variety of settings.  Social Workers/Mental Health Counselors plan, implement, coordinate and document treatment for individuals with mental health, and substance use; or perpetrators or victims of neglect, abuse, or crime who need therapeutic intervention. They provide individual, group and conjoint psychotherapy, counseling and case management services to patients experiencing psychosocial, mental health or substance use issues, including discharge planning. They perform intakes, develop treatment plans and make referrals as part of a multidisciplinary team and acting within the SBELIH and Quannacut mission, philosophy and goals



Essential Functions:


•    Provides individual, and/or group therapy services to patients and/or their families in the outpatient mental health/SUD setting.


•     Provides education, counseling and crisis intervention to patients and/or their families.


•     Utilizes psychosocial assessment skills to deliver comprehensive social work services.


•     Formulates and implements person-centered, trauma informed treatment and discharge plans.


•    Works in collaboration with the criminal justice system, care management, including AOT and ACT to provide quality care.


Required Skills/Abilities:


•    Demonstrates understanding of discharge planning.


•    Demonstrates patient-centered patient advocacy and service excellence


•    Demonstrate excellent written and oral communication skills.


•     Demonstrates understanding of community systems


•      Demonstrates teamwork.


•      Demonstrates understanding of data collection and statistics.


•      Demonstrates ability to manage complex caseloads.


•      Computer proficiency.


•      Basic Life Support (“BLS”) required; American Heart Association


Education and Qualifications:


•         Licensed Master of Social Worker, Licensed Clinical Social Worker or Licensed Mental Health counselor. NYS LMSW, LCSW, LMHC or eligible.


•         MSW, MHC from an accredited graduate school.


•         Licensed and currently registered to practice in New York State


•         Must comply with NASW and AMHCE Code of Ethics


•         NYS OASAS CASAC preferred





Physical Demands:


•         Must be able to respond quickly in an emergent situation


•         Must be able to use stairs in an emergent situation


•         Must be able to walk, carry semi-heavy equipment and reach overhead.


•         Must have excellent communication skills both orally and written in emergent situations.


•         Must be able to fully participate in Crisis Prevention Training

Anticipated Pay Range:


The starting salary range (or hiring range) for this position has been established based on relevant experience. The above salary range (or hiring range) represents SBEILH’s good faith and reasonable estimate of the range of possible compensation at the time of posting.

Prior to start date, the selected candidate must meet the following requirements:


•       Successfully complete pre-employment physical examination and obtain medical clearance from Stony Brook Eastern Long Island’s Employee Health Services *


•       Complete electronic reference check with a minimum of three (3) professional references.


•       Successfully complete a 4 panel drug screen*


•       Meet Regulatory Requirements for pre-employment screenings.


•       Provide a copy of any required New York State license(s)/certificate(s).



Failure to comply with any of the above requirements could result in a delayed start date and/or revocation of the employment offer.



*The Human Resources department will be responsible for any fee incurred for examination.

As part of Stony Brook Medicine, Stony Brook Eastern Long Island Hospital (SBELIH) combines the best in academic and community medicine, improving access to advanced medical care for residents of the North Fork and Shelter Island. With 70 beds, SBELIH is Suffolk County’s first hospital, serving the North Fork since 1905.
