Assistance Social Worker (SW) needed for an Adult Day Health Care.

ADHC/CBAS is currently seeking a Social Worker to work at our Adult Day Health Care Center (ADHC/CBAS).

Felices Dias ADHC is a licensed community-based adult services program (CBAS) that provides services to older persons and adults with chronic medical, cognitive, or mental health conditions and/or disabilities in a coordinated program of professional and compassionate services in a community-based group setting. Services are designed to provide social and health services to adults who need supervised care in a safe place outside the home during the day. They also afford caregivers respite from the demanding responsibilities of caregiving. The Program stresses partnership with the participant, the family and/or caregiver, the primary care physician, and the community in working toward maintaining personal independence.

Part-Time, Monday through Friday, no evening hours.

DUTIES AND RESPONSIBILITIES

  • Interview and screen all referrals to determine the appropriateness of the prospective participant for a full ADHC assessment. Provide referrals for applicants who are not appropriate for ADHC.
  • As a member of the multidisciplinary team will provide assessments and reassessments of psychosocial needs of the participants and develop an individualized plan of care for each participant including short term and long-term goals.
  • Maintain signed and dated documentation of all services performed on the day of service and signed and dated quarterly progress notes.
  • Make referrals to available community resources. Maintain current roster of community resources.
  • Serve as liaison to the participant's family and other caregivers.
  • Serve as liaison to community agencies providing services to the participant.
  • Participate in community education, networking and advocacy groups serving frail and disabled older adults.
  • Initiate the development of a discharge plan at admission and update the discharge plan at each reassessment period; assume responsibility for implementation of the plan at the time of discharge from the Center.
  • Conform to all applicable policies of the Center and attend the orientation program offered by the Center.
  • Comply with all health and safety standards including licensing, fire and disaster preparedness regulations and agency policies.

SKILLS AND KNOWLEDGE

  • Knowledge of the evidence-based clinical case management of chronic conditions.
  • High level understanding of community resources, treatment options, medical necessity criteria
  • Culturally competent skills to work effectively, respectfully and sensitively within the participant and family cultural context and belief system.
  • Excellent interpersonal communication skills, written and verbal: read, speak and write English fluently.
  • Strong organizational skills and analytical abilities.
  • Basic computer skills including use of PC-based software programs and databases.

MINIMUM REQUIREMENTS

License/Certification:

  • Social Work Assistant: Bachelor’s degree in psychology, Sociology, Gerontology or related fields.
