The Portage County Health & Human Services department is hiring a full-time, 40 hours per week, CLTS Social Worker.

Starting pay is $30.73 / hr

Summary of position:
This position coordinates services to families involved in Children's Long-Term Support (CLTS) Waivers Program, Children's Community Options Program (CCOP) and Children's Special Needs Unit. It is responsible for completing assessments, service plan development, and service coordination for children with Development/Intellectual Disabilities, Physical Disabilities and Severe Emotional Disabilities and their families. This position also collaborates with community agencies and local school districts thorough the provision of case management services.

What are the primary job duties?

Assist in Service Coordination for the Comprehensive Community Services (CCS) Program

  • Conduct functional screen to determine eligibility.
  • Provide orientation to the family and service providers who are new to the program.
  • Facilitate consultation among appropriate individuals and agencies regarding eligible children
Provide Service Coordination for Children with Developmental/Intellectual Disabilities, Physical Disabilities and Severe Emotional Disabilities

  • Complete comprehensive needs assessment/functional screen to determine program eligibility.
  • Provide crisis and critical incident intervention and resolution as necessary.
  • Provide on-going supervision, monitoring, and follow-along to each assigned case.
  • Consult with identified individuals and agencies regarding service delivery.
  • Assist in the development, supervision and service provision of the Individual Service Plan to meet child and family need objectives and outcomes.
  • Assist in developing a safety plan for each child covering situations at home, in the community and at school as needed.
  • Coordinate delivery of services for the children and families while monitoring progress towards identified outcomes.
  • Provide services and resources for respective family's need.
  • Conduct home and community visits.
  • Maintain complete and accurate case file to include all required documentation, case notes, etc.
  • Authorize funding requests through CLTS and CCOP.
  • Verify service delivery in compliance with program requirements and the child's outcome.
  • Provide transitional support for program enrollment and disenrollment.
What are the minimum qualifications?

  • Bachelor degree in social work or any other human services field required.
  • Certification or eligible for certification in Wisconsin as a social worker preferred.
  • One year experience working primarily with issues pertaining to Children's Long Term Support Waivers (CLTS waivers) within the last five years required.
