Pride Health is seeking a skilled and compassionate Licensed Social Worker (LCSW/LMSW) or Discharge Planner to join a top-tier healthcare facility in Washington, D.C. This is a full-time, 13-week travel contract with a consistent day shift schedule (Monday–Friday, 8:00 AM – 4:30 PM), including occasional weekend rotation. Job Summary:

The Social Worker or RN Discharge Planner will support patients throughout their hospital stay, focusing on discharge planning, care coordination, and connecting patients to critical community resources. Ideal candidates will bring previous acute care experience and a strong background in discharge planning and case management.

Key Responsibilities:

• Conduct patient assessments and determine appropriate discharge plans.
• Coordinate post-hospital care, including home health, hospice, rehab, SNF, LTAC, or DME needs.
• Review admission criteria and ensure benefits eligibility for continued care.
• Communicate with interdisciplinary teams, patients, and families to align care plans.
• Provide education and support for patients transitioning to the next level of care.
• Ensure compliance with CMS, HIPAA, and JCAHO regulations.
• Document patient interactions and care plans in Cerner EMR system.
• Collaborate with insurance and workers comp representatives, as needed.

Qualifications:

• Licensure: DC LCSW/LMSW/LGSW or RN License

Pending licensure accepted with proof of application .

• Certification: BLS required
• Experience: Minimum 2 years in discharge planning or case management
• Experience with acute care, care coordination, DME orders, and transitions of care
• Preferred: Prior travel assignment or charge experience
• Regulatory Knowledge: Familiarity with CMS, HIPAA, JCAHO; workers compensation a plus
