The Healthy Capital District serves over 15,000 people annually to access the health services they need and other social services. We support people of all ages and backgrounds to obtain subsidized health insurance coverage and overcome barriers to care. This is done through our social service directories, referral assistance, insurance enrollment, providing dental services to students in schools, and training of Certified Peers and Community Health Workers. We also create public health needs assessments to advance public health planning throughout the region.
Job Purpose
The Health Care Advocate will assist residents of the Capital Region in obtaining services related to health care products, long-term home care, mental health, substance use disorders, and other needed social services.
Duties and responsibilities
Under the direct supervision of the Director of Enrollment Services, the Health Care Advocate will perform the duties associated with providing support and enrollment for high need communities, seniors, and others. They will perform duties associated with applying for the Aged, Blind, Disabled programs, through local Departments of Social Services, in addition to providing individual counseling, community presentations on navigating health care needs and insurance. Duties consists of:
- Scheduling appointments and sending appointment reminders
- Conducting Face to Face interviews with clients
- Enrollment assistance for health insurance, Long Term Managed Care plans or Health and Recovery Program;
- Maintain contact with client from start of application through enrollment; completing follow-ups when necessary
- Interpreting plan options and financial eligibility for applicants
- Perform Managed Care education and assist with plan selection
- Assist clients in recertification process, problems with coverage, when necessary, support with filing complaints on behalf of client.
- Adhere to all policies, procedures and maintain strict client confidentiality.
- Provide, community outreach, informing community organizations about HCD services, and distributing marketing materials. Liaison for offsite locations and HCD.
- Ability to travel occasionally to local and regional community events, some evenings and weekends required.
Qualifications
Qualifications include:
- Associates degree preferred or commensurate experience
- Organizational skills, patient, persistent, accurate
- Experience working with low income and underserved populations (multicultural awareness)
- Excellent Communication skills
- Reliable transportation
- Spanish or fluency in languages other than English a plus
- Occasional availability on evenings and weekends a plus
- Experience with Microsoft Office beneficial
