How You'll Make a Difference: 

Arkos Health is seeking a Skilled Nursing Facility (SNF) Care Coordinator. The Skilled Nursing Facility Care Coordinator manages patient care at admission. The SNF Care Coordinator keeps close contact with SNF Care Manager throughout patient’s stay to monitor progression and facilitate PCP and specialty follow up. They provide oversight of SNF patients to appropriate utilization of SNF days.

Essential Duties and Responsibilities:

• Monitor and coordinate HIE and ER alert notifications, covering the days, evening and weekend hours
• Appropriately identify member needs through in-coming HIE alert and chief complaint notifications based on available Arkos programs and solutions
• Contacts and coordinates with ER staff, offer appropriate Arkos services (acute NP visits, ICM, TCM, Home Health, DME, etc.) based on data collection from ER
• Coordinates ER to SNF admissions, facilitating coordination to available beds and acceptance from SNF admission staff
• Monitor and clear alerts from HIE and document appropriately in member chart
• Accurately chart and notify internal staff through the referral portal for member needs, status and placement
• Full time role, weekends shifts included
  
  

Qualifications/Skills:

• LPN License Required
• After admission, initiate contact with visit 24-48 hours of admission and introduce services to patient
• Review admission orders and medical records
• Send recommended length of stay to health plan within 3 days of admission for continuation of SNF stay authorization
• Assist facility CM and SW teams in discharge planning by providing resources available through our organization
• Target to get patients back in their home setting, with the highest levels of care available
• Identify discharge needs (DME, assisted living, SW, ICM, PCP/specialty follow up)
• Upon discharge, ensure proper orders are in place and all additional home services have been arranged (HH, Mobile Physician, and NP)
• Establish discharge plan and home-based plan at the time of admission to SNF
• Identify appropriate clinical service program for patient following discharge (TCM/ICM/PC/NP)
