This includes connecting individuals with behavioral health services and other essential resources. The role involves in-person jail outreach and community support, with administrative tasks being performed remotely or in an office setting. Funded by CareOregon, the position focuses on providing comprehensive care coordination from a health plan perspective.As a newly hired Clinical Services Specialist, you will receive a one-time New Hire Recruitment and Retention Incentive payment of up to $1,500 on your first paycheck. The incentive will be prorated based on the position's FTE status.As the Jail Transition Care Coordinator - CSS , other duties will include:Care CoordinationProvide in-person care coordination, including jail outreach, community meetings, and telephonic support, to members with complex behavioral health needs.Coordinate with treatment teams to assess and intervene in mental health crises, determine the need for specialized consultations, and ensure treatment recommendations are understood.Assist members transitioning from jail to community care providers, facilitate communication between involved parties, and ensure care plans are shared.Coordinate multidisciplinary teams, participate in regional meetings, and address cultural factors in care coordination.Jail Care Coordination is a time limited support and transition to longer term Care Coordination programs will be an important element of this position.Apply complex rules and guidelines to minimize county risk; facilitate high-conflict meetings with the member, family, police, parole/probation officers, health care providers, and agencies; provide recommendations or input on cases regarding guardianship, conservatorship, and other civil rights issues; identify problems and develop action plans and treatment strategies.Screening, Assessment and Care PlanningScreen and assess referred members to determine eligibility for Jail Care Coordination Services.Utilize screening tools, clinical consultations, and information from various sources to make determinations.Conduct comprehensive assessments, create individualized care plans, and lead interdisciplinary teams to establish and monitor member goals.Ensure cultural factors are considered throughout the process.Documentation and ComplianceMaintain timely and accurate documentation about each member.Document all services provided and complete service note within 24 hours, no later than 72 hours from the time of providing the service.Provision of Care Coordination and Documentation of activities adheres to the Oregon Administrative Rules for Care Coordination and BHD policy for documentation.Maintain tracking and data entry for program reporting and evaluation.
