General Purpose:In a few brief sentences summarize the primary duties and responsibilities.
Provide direct supervision to Habilitation Specialists and/or Direct Support Professionals (Rehabilitation Technicians) in Community Prevocational and Day Habilitation Services for individuals with Intellectual and Developmental Disabilities. Coordinate & ensure service delivery for a case load of up to 30 individualswith I/DD.Additionally overseeing Habilitations Specialist’s implementing of Person-Centered Practices to the program in its entirety. The supervisor is responsible for engaging each individual in a Person-Centered Planning process to identify their goals and implement services and activities that will help them achieve their highest level of independence while providing leadership to Habilitation Specialists and Direct Support Professions to ensure quality services throughout program.The supervisor splits his/her time between providing on-site guidance and leadership and taking care of various administrative issues as described in the essential function section.Complete all required documentation efficiently & to employers & funding source's standards. The supervisor is responsible for coordinating group & individual scheduling of activities and services, regularly visit program, volunteer sites and regularly complete assessments and/or updating Staff Action Plans.
Essential Functions: Describe the intrinsic job duties that define the core responsibilities of the job.

Complete required documentation (including Life Plans, Monthly Case Notes, Person Centered Plans, Prevocational Assessments, DDP1’s, Incident Reports, and other required documentation) to funding source and oversight agency standards within specified timeframes.

Ensure Individual's on caseload and in program meet monthly attendance goals. Prepare monthly attendance report for Program Manager.

Engage each individual in Person Centered Planning.Use their Person Centered Plan and feedback as the basis for developing Staff Action Plans, Employment Plan, Goals, Staff Supports & Services.

Creategroup & individual monthly schedule. Each individual's goals, interests, needs, and skills must be taken into account when developing the schedules. * Supervisor is required to ensure schedule development if Senior Technician is not assigned.

Solicit feedback, observations, insights, & recommendations from the individual, Rehabilitation Technicians, & Job Developer regularly on individual's progress & changes to make to schedules & Staff Action Plan/Employment Plan.

Ensure Person Centered Planning and Practices are carried out with Habilitation Specialists and Direct Support Professionals for all program participants.

Responsible for submitting & balancing Petty Cash reports, tracking Metro Card distribution & reconciliation, submitting monthly transportation reimbursements, and conducting monthly fire drills at offsite locations.*Supervisor must fulfil this is Senior Technician is not assigned.

Visit hub sites & volunteer / internship sites to supervise activities, evaluate individual's progress, and monitor the quality of services/ activities and interactions between Rehabilitations Technicians and individuals.

Responsible for ensuring Rehabilitation Technicians provide daily services as written in the individual's Habilitation Plan and accurately enter time & attendance and services provided to individual'son a daily basis.

In accordance with OPWDD and agency requirements modifies his / her own communication to ensure understanding & respect when interacting with individuals; including modifying tone of voice and use of verbal & nonverbal techniques to meet the individual's communication needs.

Recruitment of appropriate individuals into program, by doing outreach to Schools, Transition Coordinators, MSC’s etc. in order tomaintainappropriate number of clients in program.

Liaise and troubleshoot with Transportation Company.

Participate in annual Life Plan meetings.

Provide oversight, guidance and discipline for rehabilitation techs.

Responsible for hiring and disciplinary actions when needed.

Works directly with QA dept. to ensure adherence to maintaining charts in audit ready condition.

Responsible for uploading and working with CHOICES & IRMA.

Responsible for ADP T&A Supervisor

Responsible for submitting requisitions in D35 * New Line combining all ADMIN assignments

Responsible for overall site operations and safety.

Qualifications/Basic Job Requirements: Describe the skills, knowledge and abilities required. Include minimum educational and experience requirements and preferences.
Education: Bachelor degree from an accredited school in related field preferred and/or combination of related work experience.
Experience: One year of work experience in mental health, vocational rehabilitation, or case management preferred.

Address performance issues with Rehabilitation Technicians including punctuality, absences, communication issues and relate any issues to Supervisor.Create Performance Improvement Plans when necessary.
