The Crisis Care Coordinator is a highly responsible service position. Crisis Care Coordinator will work with behavioral health clinic staff, other agencies, community organizations, individuals/families in services, and the general public. This position will be located in Uvalde County, Texas and will be required to provide face to face services during operational hours and may require after-hours services. This position provides and ensures that 24-hour on call crisis services are readily available. The delivery of services will be based on the needs of the community and individuals experiencing a psychiatric emergency. The crisis assessments may be conducted in the individual’s home, local hospital, jail, and school settings in addition to the clinic. The Crisis Care Coordinator position is responsible for assessing individuals in crisis situations, developing individualized crisis treatment plans, and providing crisis follow up contacts.

The Crisis Care Coordinator must provide timely and accurate documentation which supports diagnosis and outcome-based services. The Crisis Care Coordinator exercises extensive independent judgment and does require flexibility in their schedule for day shift, after hours, and weekends. The Crisis Care Coordinator will also help to maintain data, which will be used to plan and make projections for current and future services to meet outcomes, set by the state contract. The Crisis Care Coordinator works cooperatively and collaboratively with communities and inter-agency staff and reports to the Clinic Director or Crisis Team Lead.

You will get to:

You’ll definitely need:

  • Bachelor’s Degree from an accredited college in social, psychology, behavioral or human services
  • Must meet QMHP guidelines – candidate must submit copies of college transcripts
  • One year of experience working on a crisis team or conducting mental health crisis assessments, preferred
  • To complete and maintain both CPR certification and Prevention and Management of Aggressive Behavior training.
  • To have a valid Texas State Driver’s License.
  • To have current automobile insurance and the ability to travel to individuals when necessary.

  • Employ excellent verbal and written communication skills to engage and report effectively.
  • Maintain a well-organized schedule and travel as necessary to ensure completion of duties.
  • Maintain accurate and timely collaborative documentation with adherence to HIPPA regulations.
  • Sustain a calm demeanor when under pressure to handle sometimes difficult situations.
  • Provide exemplary customer service and care to individuals with their best interest at heart.
  • Work autonomously to complete assignments within community and HHSC standards with a high level of quality.
  • Work cooperatively with others and be punctual and consistent in attendance because your people will rely on you!

We can pay you:

Up to $61,140 per year and benefits eligibility. This annual salary includes a base salary plus a critical access stipend FY2023
