Responsibilities
- Communicate with the patient’s treatment team and have a thorough understanding of the patient’s behaviors, current presentation, and future treatment goals
- Coordinate care with family, community, outpatient providers, and inpatient team via meetings with the individual responsible physician
- Carry a caseload, meet with patients daily, and maintain accurate document
- Master’s degree from an accredited college or university in social work, mental health, nursing, or related degree preferred
- A minimum of three (3) years’ experience in a healthcare setting or managed care company, with experience in patient assessment, treatment planning, utilization management, and/or case management
- Licensure: LMSW, LCSW, LAC, LPC, LIASC, LMFT, or other healthcare – related licensure required or the ability to obtain within 6 months of hire
