Responsibilities


Valley Hospital Phoenix is seeking a dynamic and talented Social Work Therapist (MA/MSW) to join our team of compassionate, dedicated professionals.


The Social Work Therapist (MA/MSW) works closely with UR department on obtaining authorizations from insurance companies throughout the patient’s stay, completes biopsychosocial assessment, assists in coordination of care to lower-level treatment interventions and assists the treatment team in understanding the barriers to discharge planning.


The Social Work Therapist (MA/MSW) advocates for the patient’s access to services during treatment coordination of care with family, community, outpatient providers, and inpatient team meetings with individual physician contact. The MA/MSW will carry a caseload, meet with patients daily and provide accurate documention of the meeting. They are responsible for completing biopsychosocial assessments, treatment plans, referrals, family engagement, staffing’s, and coordinating with outpatient providers.

Duties and Responsibilities:


  • Communicate with the patient’s treatment team and have a thorough understanding of the patient’s behaviors, current presentation, and future treatment goals
  • Coordinate care with family, community, outpatient providers, and inpatient team via meetings with the individual responsible physician
  • Carry a caseload, meet with patients daily, and maintain accurate document


Location: 3550 East Pinchot Avenue, Phoenix, AZ


Schedule: This is a full-time, Tuesday – Saturday; 8am – 5pm

Qualifications


  • Master’s degree from an accredited college or university in social work, mental health, nursing, or related degree preferred
  • A minimum of three (3) years’ experience in a healthcare setting or managed care company, with experience in patient assessment, treatment planning, utilization management, and/or case management
  • Licensure: LMSW, LCSW, LAC, LPC, LIASC, LMFT, or other healthcare – related licensure required or the ability to obtain within 6 months of hire
