CareOne Home Health is seeking an Intake Coordinator for our growing home health business in Salida, CA. The Intake Coordinator is responsible for coordinating all aspects of the patient intake process including maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization processes. Collaborates with the Director of Patient Care Services and Account Executives to ensure timely and appropriate admissions

This position will be based out of our Salida, CA office and is full time with on call rotating weekends.

Primary responsibilities include:

  • Coordinates daily patient referral and intake operations with DPCS and Account Executives.
  • Enters patient information into EMR, including detailed referral information as it relates to patient transition to home health care.
  • Maintains patient confidentiality and protects operations by monitoring confidential information.
  • Ensures compliance with all state, federal, and CHAP referral/intake regulatory requirements.
  • Maintains positive working relationships with current and potential
    referral sources.
  • Ensures seamless transition of patients to home health care by providing patient education and preparation for home health care and coordination of care with multiple service providers.
  • Participates in insurance verification and authorization processes.
  • Assists in assessing DME needs and pacing orders for approved DME.
  • Gathers, collates, and reports referral statistics including key customer referral trends.
  • Maintains comprehensive working knowledge of community resources and assists referral sources in accessing community resources should services not be provided by the organization.

Requirements:

  • 1 year related experience, home health experience required
  • Proficient on all Microsoft Office software (Word, Excel, PowerPoint, Outlook)
  • Experience with Kinnser (WellSky) EMR preferred
  • Experience with Allscripts preferred
  • Bilingual Spanish – Speak, read and write preferred
