Position Summary:

The Care Manager is a key member of the CTI component, responsible for delivering intensive case management services to individuals with complex needs, including those transitioning from institutional care. The Care Manager will work collaboratively with discharge teams, community providers, and internal staff to ensure seamless service delivery. This role is crucial in coordinating housing, mental health services, and developmental disability services, while maintaining continuous care monitoring and adapting plans as needed to reflect the changing needs of individuals.

Position Duties:

Duties include, but are not limited to, the following:

  • Provide intensive case management services, ensuring that individuals have access to necessary resources and support for community integration.
  • Coordinate housing, link individuals to mental health and I/DD services and manage ongoing care needs.
  • Collaborate with hospital discharge teams, community providers, and CPU clinical staff to facilitate seamless transitions.
  • Participate in on call rotation to provide telehealth support as needed to CTI referral sources and participants
  • Monitor client progress, adjusting care plans to reflect evolving needs, and ensure that care coordination aligns with individual recovery goals.
  • Advocate for clients' needs, empowering them to navigate systems and access services that support long-term stability and recovery.

Requirements:

  • Bachelor’s degree in social work, psychology, or a related field required.
  • Minimum of two (2) years of experience in care management, preferably serving mental health and I/DD populations.
  • Strong organizational and problem-solving skills, with the ability to manage complex cases and coordinate across systems.
  • Full compliance with all applicable laws, regulations, policies, and procedures.
  • Adherence to the agency’s Corporate Compliance Program, including standards of conduct, NYS and federal False Claims Acts, whistleblower protections, and mandatory reporting requirements.
  • Completion of required corporate compliance training annually and full cooperation with compliance investigations and reviews.

License:

  • Valid New York State Driver’s License.
  • Must meet agency requirements for insurability


Location:
Multiple, including but not limited to, CPU Hudson Valley Office, Mahopac Transitional Residence, and community-based locations, including hospital referral sites throughout the Hudson Valley.
