The Five Towns Premier Rehabilitation and Health Care Center is seeking a full time social worker to join our team! Days are Monday-Friday. Competitive salary and benefits. Candidates should be experienced in discharge planning, long term care/SNF and possess good verbal and written skills. Customer service oriented and being able to work as part of a team is a must!
