TAY Case Manager provides evaluation, consultation and interventions as well as additional services of Supported Employment and Supported Education with youth and young adults in the home, school, workplace and community settings.  Also participates in conducting psychosocial programming with this population as part of a clinical team.EXAMPLES OF WORK PERFORMED:Works with youth and young adults in mental health, home, community and/or office settings.Provides case management, skills training, employment assistance and crisis intervention for youth and young adultsProvides assessment, evaluation and recommendations for interventions and treatment planningRefers youth, young adults and families to appropriate social welfare and health agencies to obtain assistance as neededPerforms other related duties as requiredKnowledge, Skills, and Essential FunctionsExcellent written communication and computer keyboarding for inputting clinical documentationExcellent verbal communication with consumers, staff, and community providersTime management skills required to meet service goalsLocal travel 50% to consumer homes, school, transporting clients, and other community settings: Regional travel 5%. Stooping, bending, sitting, standing, keyboarding for prolonged periods, and lifting 30 lbs.Ability to be CPR and PMAB certifiedValid driver's license, liability insurance, and reliable transportation required. 30% local travelEducation and ExperienceBachelor's or master's in social work, psychology or related field with ability to obtain Qualified Mental Health Professional credentialing1 year of case management and knowledge of community resources preferred but not requiredBilingual Preferred   Hours: M-F 8AM-5PM with variations as neededSalary:  $45,000-$52,965 Annual DOQPosition: 753  Heart of Texas Region Behavioral Health Network strives to offer competitive compensation and a comprehensive benefits package that includes:Benefits: Employer paid Health Insurance Employer paid Dental Insurance Employer paid Life Insurance Employer Paid Short Term Disability Employer Paid Employee Assistance Program Payroll Direct Deposit 12 paid Holidays per yearExcellent 401-K Retirement Plan (Center will contribute 12% of employee earnings)**               **Certain Requirements Apply**Generous Paid Time Off (PTO) - begins after 3 months of full-time employment
