TAY Case Manager provides evaluation, consultation and interventions as well as additional services of Supported Employment and Supported Education with youth and young adults in the home, school, workplace and community settings. Also participates in conducting psychosocial programming with this population as part of a clinical team.EXAMPLES OF WORK PERFORMED:Works with youth and young adults in mental health, home, community and/or office settings.Provides case management, skills training, employment assistance and crisis intervention for youth and young adultsProvides assessment, evaluation and recommendations for interventions and treatment planningRefers youth, young adults and families to appropriate social welfare and health agencies to obtain assistance as neededPerforms other related duties as requiredKnowledge, Skills, and Essential FunctionsExcellent written communication and computer keyboarding for inputting clinical documentationExcellent verbal communication with consumers, staff, and community providersTime management skills required to meet service goalsLocal travel 50% to consumer homes, school, transporting clients, and other community settings: Regional travel 5%.
