Medical social worker

· Master's degree in Social Work, OR

· Master's degree in psychology, gerontology, or counseling from an accredited school AND has 1 year of health, or long-term care services

· Provide Assessment and create care plans for clients

· Provide wellness checks to clients / caregivers

· Coordinate with IHSS services as needed

· Offer referrals and resources to clients and caregivers

· Mental health interventions as needed

· Develop and lead support groups

· Contribute as an active member of the multidisciplinary team

. Bilingual (Spanish, Chinese) preferred

Job Type: Full-time

Pay: $30.00 - $32.00 per hour
