Overview
We are seeking a reliable, organized, and clinically thoughtful California-licensed RN to serve as a private RN care manager / patient advocate for a 92-year-old male recovering from hip fracture surgery and transitioning through hospital and skilled nursing facility care in the Los Gatos / South Bay area, eventually to home care.

This is not a passive sitter role and is not intended to replace hospital or SNF staff. The RN’s role is to provide continuity, observation, documentation, family communication, and appropriate escalation of concerns while working collaboratively with facility staff.

We are ideally looking for:

One primary weekday RN: Monday–Friday, approximately 8 hours/day and/or One weekend RN: Saturday–Sunday, approximately 8 hours/day

The initial assignment is expected to last 1-3 months with the possibility of extension or a permanent role depending on mutual fit and family needs.

Responsibilities

  • Provide in-person observation and care coordination at hospital and/or skilled nursing facility.
  • Track daily condition, including mental status, alertness, pain, sleep, appetite, hydration, bowel/urine status, mobility, therapy tolerance, and concerning symptoms.
  • Monitor and document facility-administered medications, lab trends when available, oxygen concerns, sedation, confusion/delirium signs, fall risk, and other relevant changes.
  • Communicate appropriately with hospital/SNF nurses, charge nurses, case managers, therapists, and physicians when concerns arise.
  • Track PT/OT participation and objective rehab progress, including transfers, walking distance, assist level, and barriers to progress.
  • Provide concise written daily updates to the family.
  • Maintain a running issue list, follow-up questions, and handoff notes.
  • Coordinate observations from private caregivers, if present.
  • Support safe transitions from hospital to SNF and eventually from SNF to home.

Experience

  • Current California RN license in good standing.
  • Experience with older adults, geriatric patients, SNF care, hospital discharge, home health, hospice, orthopedic recovery, case management, or similar settings.
  • Strong documentation and communication skills.
  • Calm, professional, collaborative style.
  • Ability to recognize and escalate concerning changes in condition.
  • Reliable transportation to Los Gatos / South Bay facilities.
  • Ability to commit to a consistent schedule for at least several weeks.

Preferred experience

  • Geriatric care management.
  • RN case management or discharge planning.
  • SNF, rehab, orthopedic, home health, or hospice nursing.
  • Experience with patients age 85+.
  • Familiarity with delirium risk, fall prevention, post-op rehab, anemia monitoring, medication side effects, bowel regimens, hydration, and safe discharge planning.
  • Prior private patient-advocacy or family care-coordination work.

Schedule

We are open to either of the following:

  • Weekday RN: Monday–Friday, approximately 8 hours/day.
  • Weekend RN: Saturday–Sunday, approximately 8 hours/day.

Strong preference for consistent weekly availability rather than rotating occasional shifts.

Pay

$70 to $120 / hour depending on experience. Higher compensation may be considered for an exceptional RN with strong geriatric care-management, hospital-to-SNF transition, or patient-advocacy experience.

Important scope clarification

The RN is expected to observe, document, coordinate, and advocate. The RN should not override hospital/SNF staff, administer medications, move/transfer the patient, adjust medical equipment, or perform facility nursing duties unless expressly permitted by the facility and within applicable scope and policies.

Ideal candidate

The ideal candidate is a mature, detail-oriented RN who can become the family’s “clinical quarterback”: someone who notices patterns, writes clear updates, communicates calmly with staff, and helps ensure that important issues are not lost across shifts or care transitions.

Pay: $70.00 - $120.00 per hour
