MSW Senior Director of Care Management Orlando, FloridaThe MSW Senior Director of Case Management serves as a subject matter expert in Case Management, demonstrating solid leadership, trust-building, team building, and Change Management abilities to drive departmental success and initiatives. The Senior Director is responsible for driving seamless Care Coordination, Discharge Planning, Utilization Management, and Transitions of Care while partnering closely with physicians, nursing leadership, and executives to optimize patient outcomes, reduce readmissions, and ensure regulatory compliance.Qualifications:LMSW with acute hospital nursing experience requiredDirector of Case Management in an Acute Hospital Setting requiredMaster’s Degree requiredCertification in Case Management (CCM/ACM) preferred Responsibilities: Oversees departmental recruitment and retention activities to ensure adequate staffing and leadership in Care ManagementActively engages with post-acute care entities to improve patient care transitions and decrease readmissionsLeverages technology change management to improve staff efficiencies, drive positive patient outcomes, and maximize the tools Care Managers utilizeDirects daily operations of the Care Management department, including discharge planning, care coordination, progression of care, and social servicesEnsures compliance with CMS Conditions of Participation for Discharge Planning and related federal and state regulationsParticipates in Corporate Care Management meetings, cascades information, and implements strategies with standard work and processesDevelops and oversees implementation and compliance with Care Management policies and standard operating proceduresCollaborates with interdisciplinary hospital leaders and executives to achieve organizational goalsDevelops and meets operational budget performance with oversight to assure capital and fiscal accountabilityPartners with Utilization Management for the Utilization Review Committee and day-to-day operationsIdentifies improvement opportunities and developments, implements, and monitors action plansEducates and mentors facility Care Management staff and interdisciplinary teams on data, outcomes, and opportunitiesHow to Apply: Interested candidates, please submit your resume to Michelle Boeckmann at Michelle@CMRecruiter.com.
