We’re home to an interdisciplinary team of experts – ranging from primary care providers to mental health specialists and gender-affirming surgeons.What you will do as anRN Care Coordinator:Coordinates care across the health care continuum within designated sub-specialty population(s)Serves as a single point of contact by facilitating and coordinating transitions in care within and between care settings, providers, and servicesCompletes pre-visit planning and post-visit wrap-up and determines vital follow-up appointments and/or treatments to optimize patient visitsFacilitates proactive disease management and preventative health care using evidence-based practiceRooms patients for the surgeon(s) as needed, complete clinic readiness including records review and provides administrative support as needed.What you will need:Current Minnesota RN license3+ years of Clinical Nursing experienceBLS certificationDemonstrated experience in patient advocacy and education, coaching, and proactive health care management along with EMR/digital literacyLocation: M Health Fairview Clinic and Surgery Center - 909 Fulton St. SE Minneapolis, MN 55455Hours: 40 hours/weekBenefits: This is a fully benefit eligible position: Competitive wages, Healthcare (including vision & dental), 401K, parking & tuition assistance and more!Compensation:77,735.00 - 124,369.00 USD AnnualAt M Physicians we believe in pay transparency and equity.
