Hospice Medical Social Worker LMSW

Job Overview
We are seeking a compassionate and dedicated Medical Social Worker to join our healthcare team. The ideal candidate will possess a strong background in social work, with experience in providing support to patients dealing with various medical conditions. This role involves assessing patient needs, developing care plans, and coordinating services to ensure comprehensive care for individuals, particularly those facing challenges such as a terminl illness, senior care requirements, and behavioral health issues.

Duties

• Conduct thorough patient assessments to identify emotional, social, and environmental factors affecting their health.
• Develop and implement individualized care plans based on patient needs and goals.
• Provide counseling and support for patients dealing with PTSD and other behavioral health issues.
• Utilize cognitive behavioral therapy techniques to assist patients in coping with their conditions.
• Maintain accurate medical documentation in compliance with ICD-10 standards.
• Collaborate with healthcare professionals to coordinate case management services.
• Advocate for patients' needs within the healthcare system and connect them with community resources.
• Monitor patient progress and adjust care plans as necessary to ensure optimal outcomes.

Qualifications

• Master’s degree in Social Work (LMSW) from an accredited program.
• Valid state licensure as a Clinical Social Worker (LCSW) or equivalent.
• Experience in PTSD care, senior care, or behavioral therapy preferred.
• Proficient in patient assessment and medical documentation practices.
• Strong interpersonal skills with the ability to communicate effectively with patients, families, and healthcare teams.
• Knowledge of case management principles and practices.
• Ability to work independently as well as part of a collaborative team environment.
