• Collect and document data related to patient progress, treatment outcomes, and service utilization for clinical research purposes.
• Support patients in navigating community resources such as assisted living facilities, home care services, or specialized programs for dementia.

Experience

• Proven experience in hospice care or end-of-life support services with a strong understanding of hospice protocols.
• Prior inpatient or outpatient clinical experience in hospitals, nursing homes, assisted living facilities, or community health programs is highly desirable.
• Skills in crisis management, diagnostic evaluation, case management, and working effectively within HIPAA guidelines are essential for success in this role.
