Virtual Care Coordinator – Onsite (Contract)

Location: New York, NY 10017
Schedule: 03:00PM - 11:00PM | 8 hrs/day | 40 hrs/week

Job Summary

Seeking an experienced Virtual Care Coordinator to support hospital-based telehealth and remote patient monitoring programs. This is an onsite role requiring strong clinical judgment, communication, and technology skills.

Key Responsibilities

• Monitor patients virtually for safety and clinical needs
• Support telehealth consults and scheduled visits
• Escalate clinical or safety concerns to RN/physician
• Provide real-time technical support to clinical staff
• Maintain accurate documentation and patient privacy

Requirements

• Minimum 3 years direct patient care experience
• Experience with telehealth or remote monitoring
• Strong communication and computer skills
• Experience with EMR systems

Education

• Associate or Bachelor’s degree in a health-related field (preferred)

Pay: $25.00 per hour

Experience:

• Virtual Care Coordinator: 2 years (Required)
• Telehealth: 2 years (Required)
