Social Worker

Summary

Job Summary: Provides social services and discharge planning support for PRMC in collaboration with the interdisciplinary healthcare team throughout a patient’s hospitalization. Assists patients and families through psychosocial assessments, crisis intervention, advance care planning, and coordination of discharge needs. Connects patients with community resources and supports them in managing the financial, emotional, and social challenges related to healthcare and recovery. May also assist with emergency room and clinic visits as needed. Delivers psychosocial support to individuals, families, and groups coping with chronic, acute, or terminal illnesses.

Job Qualifications: Master (LMSW) or Bachelors (LBSW) in social work from a university or college accredited school of social work. Requires 2–3 years' experience in a hospital setting.
