Liberty Healthcare is hiring for a full-time Lead Options Counselor position.
As a Lead Options Counselor, you will be an active contributor with a statewide Aging and Disability Resource Center system which supports older individuals and people who are living with disabilities.
Program Objectives Include
- Functioning as the “go to” resource for people who are seeking information on long term care and other community-based services and supports
- Providing person-centered planning in a way that supports informed decision making and the ability to secure services which will enable people to live as independently as possible
- Leading a team of six field-based Options Counselors
- Ensuring a significant number of Kansans receive high quality options counseling services
- Data analysis and reporting
- Participating in quality assurance initiatives
- Providing guidance and consultation on complex matters
- Meeting with participants and their families in-person and virtually
- Assessing participant’s needs, providing information, and answering questions
- Connecting people with appropriate services and supports
- Documenting interactions, services, and relevant information in computer systems
- Traveling to communities throughout Kansas and participating in onsite meetings
- Creating partnerships with stakeholders
This position will require you to travel to communities throughout Kansas for in-person meetings and allow you to partially work from your home office.
As a full-time employee of Liberty Healthcare in this role, you can expect:
- Salary starting at $60,000
