We are seeking a dedicated and experienced Associate Social Worker with over 12 months of professional practice to join our dynamic healthcare team. This role offers an exciting opportunity to provide compassionate, comprehensive social work services within a multidisciplinary setting. You will play a vital part in supporting individuals and families through clinical assessments, counseling, case management, and crisis intervention, ensuring they receive the highest quality of care. Your expertise will contribute to improving patient outcomes and fostering a supportive environment for diverse populations, including children, adults, and seniors with complex needs.
Duties
- Conduct thorough patient assessments, including diagnostic evaluations and intake interviews, to identify individual needs and develop personalized care plans.
- Provide individual, group, and family counseling utilizing evidence-based practices such as cognitive behavioral therapy (CBT), psychodynamic therapy, behavioral therapy, and applied behavior analysis (ABA).
- Deliver specialized services for populations with developmental disabilities, autism spectrum disorder, PTSD, Alzheimer's disease, geriatrics, and other mental health conditions.
- Coordinate care through case management activities, including home visits, community outreach, and collaboration with healthcare providers to ensure continuity of care.
- Maintain accurate medical documentation in electronic medical records (EMR) systems while adhering to HIPAA regulations and medical terminology standards.
- Facilitate crisis intervention and crisis management strategies to support patients experiencing mental health emergencies or behavioral crises.
- Engage in clinical research activities such as data collection and analysis to support ongoing program improvements and evidence-based practices.
Skills
- Proven experience in psychotherapy techniques including cognitive behavioral therapy (CBT), psychodynamic therapy, addiction counseling, and trauma-informed care such as PTSD treatment.
- Strong working knowledge of medical documentation standards, ICD coding (ICD-9/10), CPT coding, and familiarity with HIPAA compliance for patient privacy.
- Excellent skills in patient assessment, diagnostic evaluation, and intake procedures within hospital or community settings.
- Ability to utilize EMR systems efficiently for documentation and data collection purposes.
- Experience working with children, individuals with autism or disabilities, and providing child & family counseling in diverse environments such as hospitals or community clinics.
- Competence in crisis management techniques and group therapy facilitation tailored to behavioral health needs.
Pay: $40.00 per hour
