Qualifications
Master’s Degree in Social Work (MSW)
Job description:
- Conducts home visits, intake assessments, scheduling, evaluations, case management, crisis intervention and follow- up services for Home Sharing and Respite Care Program.
- Handles intakes for new clients for the Home Sharing and Respite Care Program
- Schedule guest and host appointments
- Participates in the service delivery components of Home Sharing Program, follow-up, outreach, entitlements and benefits assistance, advocacy, etc.
- Assists with assessment and matching of clients with extensive follow-up
- Prepares and organizes case files
- Answer program related telephone inquiries and provides information and referral services
- Develops and maintains ongoing outreach with legislators, local media, community resources
- Makes follow up phone calls to all Home Sharing clients after a match is made
- Assists Home Sharing and Respite Care Program Director with record keeping and in insuring timely preparation of all required reports
- Verifies all references for Home Sharing and Respite Care applicants
- Assists with follow up to physicians regarding Respite Care Program medical form
- Other tasks as assigned
