Qualifications
Master’s Degree in Social Work (MSW)

Job description:

• Conducts home visits, intake assessments, scheduling, evaluations, case management, crisis intervention and follow- up services for Home Sharing and Respite Care Program.
• Handles intakes for new clients for the Home Sharing and Respite Care Program
• Schedule guest and host appointments
• Participates in the service delivery components of Home Sharing Program, follow-up, outreach, entitlements and benefits assistance, advocacy, etc.
• Assists with assessment and matching of clients with extensive follow-up
• Prepares and organizes case files
• Answer program related telephone inquiries and provides information and referral services
• Develops and maintains ongoing outreach with legislators, local media, community resources
• Makes follow up phone calls to all Home Sharing clients after a match is made
• Assists Home Sharing and Respite Care Program Director with record keeping and in insuring timely preparation of all required reports
• Verifies all references for Home Sharing and Respite Care applicants
• Assists with follow up to physicians regarding Respite Care Program medical form
• Other tasks as assigned

Pay: $61,800.10 per year
